Nutrition Counseling

Outpatient Dietitian Services

Our outpatient dietitian will provide you with individual consultations to offer personalized recommendations and plans to assist with a variety of nutrition-related diseases and concerns, including but not limited to:

  • Cancer
  • Diabetes
  • Gastrointestinal disorders
  • Heart health
  • Kidney disease
  • Polycystic Ovarian Syndrome
  • Pregnancy and breastfeeding
  • Vegetarian eating
  • Weight management

Education and resources will be based on the latest research in nutrition as it relates to your health. We also provide practical advice on the following:

  • Dietary adequacy
  • Easy to prepare recipes
  • Eating-out tips
  • Interpreting food labels
  • Meal planning
  • Strategies for change

Initial Appointment

During your visit, our outpatient dietitian will make an evaluation through discussion of your current food consumption, medical background, lifestyle, family and work as well as taking some physical measurements. Relevant dietary education, including handouts, will be provided and goals will be defined. The dietitian, you and your primary care physician (if appropriate) will collaborate to create an individualized treatment plan. A letter to your referring health professional will also be sent when requested.

Review Appointment

During your review appointment, your progress and any difficulties you may be having implementing your goals are discussed. Questions you have will be answered, and further education may take place – this could be regarding a new topic or expanding on an existing issue. Practical advice and encouragement are always available for your support, with new goals set as necessary to keep you motivated.

Referrals

A written doctor's referral is required.
